Victim identified in fatal Dodge County accident

July 12, 2017 – Dodge County, WI – On July 11 at 2:17 am, the Dodge County Sheriff’s Office responded to a motor vehicle versus semi-tractor with box trailer crash on northbound US 151 near Gunn Road, in the Township of Calamus, Dodge County, Wisconsin.

Preliminary investigation shows that Levi J Nielsen, 25, of Evansdale, Iowa was operating a 2006 Ford pickup truck towing an enclosed trailer with race car inside northbound on US 151 in the right/outside lane approaching the rear of a northbound 2014 Volvo semi-tractor with box trailer.

The pickup truck attempted to swerve to the left/inside traffic lane to avoid collision with the semi, but the right side of the pickup truck struck the left rear and left side of the semi-trailer.

A passenger in the pickup truck, Jessica J. Miller, 43 years old, of Evansdale, Iowa was ejected from the vehicle and pronounced dead at the scene by the Dodge County Medical Examiner.  Nielsen sustained minor injuries in the crash and declined medical treatment.  The driver of the semi, Jasvir S. Kattara, 29, of Van Nuys, California was uninjured in the crash.

Contributing factors are being investigated, but fog was not present at the time of the crash.

Assisting at the scene was:  Beaver Dam Police, Beaver Dam Fire Department and Paramedics, Dodge County Emergency Response Team and the Dodge County Sheriff’s Office Chaplain.

The crash remains under investigation by the Dodge County Crash Investigation Team and the Dodge County Medical Examiner.
