Those Mike leaves behind to cherish his memory includes his loving wife of 41 years, Sandra Survis; three children, Katherine Survis, Adam (Yvonne) Survis, and Dustin Survis; a granddaughter, Eleanor Survis; his mother, Ruth Survis; three sisters, Sheila (Frank) Bauer, Susan (Wayne) Kleinmann, and Alice (Andrew Jr.) Breit; two brothers, Daniel (Judith) Survis and Bruce (Judith) Survis; a sister-in-law, Mary Survis; many nieces and nephews; other relatives and friends.

He joins in eternal life his father, Forest Survis, and two brothers, Steven Survis and Gene Survis.
