August 6, 2021 – Town of Polk, WI – On August 5, 2021, at 8:34 p.m. the Washington County Sheriff’s Office received multiple reports of a two-vehicle crash that occurred on Cty Tk PV at Hy 45 in the Town of Polk.
It was reported that the crash involved a van that was still in traffic and a sedan that was in the ditch.
An off-duty Washington County Sheriff’s Office deputy was on scene and advised that one of the occupants of the sedan had been ejected and that the operator of the sedan was still inside of the vehicle but was unresponsive.
Washington County Sheriff’s Office deputies along with Jackson Fire and Rescue and West Bend Rescue were dispatched to the scene.
Multiple witnesses reported observing the sedan operating in an erratic manner prior to being involved in the crash. The original caller in the incident was on the phone with Washington County Dispatch attempting to report the erratic driving when the crash occurred.
The crash remains under investigation by the Washington County Sheriff’s Office Accident Reconstruction Team.
The initial investigation revealed that the sedan had exited from Hwy 45 northbound and failed to stop for the stop sign at the top of the exit ramp at Cty Tk PV. The sedan drove through the intersection and over the median that separates eastbound and westbound traffic on Cty Tk PV. The sedan then struck a westbound van on Cty Tk PV.
Traffic on Cty Tk PV was closed between Cty Tk P and the on-ramp for Hwy 45 southbound for approximately five hours for the investigation.
The operator of the westbound van, a 44-year-old Village of Lannon man, was transported to Froedtert West Bend Hospital with minor injuries. The operator of the northbound sedan, a 59-year-old Village of Greendale man, was transported to Froedtert Hospital Wauwatosa via air medical helicopter by Flight for Life with critical injuries. The passenger of the northbound sedan, a 43-year-old Village of Butler woman, was transported to Froedtert West Bend Hospital with serious injuries and was later transferred to Froedtert Hospital Wauwatosa.
Negligent operation of a motor vehicle and impairment may be factors in the crash.
