Fatal accident on I41 in Washington County | By Washington Co. Sheriff

July 9, 2018 – Town of Polk, WI – On July 8, 2018 at 7:44 p.m. the Washington County Sheriff’s Office was notified of a two vehicle crash, with injuries, on I41 south of CTH K in the town of Polk.

Sheriff’s Deputies, Slinger Police, and rescue personnel from Lifestar Ambulance were subsequently dispatched to the scene.

An on-scene investigation revealed the following: A mini-van operated by an 18-year-old male was northbound on I41 south of CTH K.

The operator of the mini-van crossed over the center median and into southbound traffic on I41.

A full size SUV struck the mini-van on the passenger side.

The SUV was occupied by 1 adult female driver and 6 juveniles. The male driver suffered life threating injuries, Flight for Life was requested and landed on scene.

CPR was started on the male driver, he was later declared deceased and not transported by Flight for Life.

The adult female and 6 juveniles were transported to local hospitals for non-life threating injuries.

Assistance was requested from Wisconsin State Patrol, Slinger Fire Department, Hartford Fire Department and Richfield Fire Department. For Traffic control and patient transport.

I41 southbound at CTH K was closed for approximately 2 hours and 45 minutes to facilitate rescue, investigative, and clean-up efforts.

The accident remains under investigation by the Washington County Sheriff’s Office.
