May 1, 2017 – Dodge Co, WI – A 68-year-old Columbus man involved in an accident last week Tuesday evening in neighboring Dodge County has died from injuries suffered in that accident.
According to the Dodge County Sheriff the accident occurred at 11:57 p.m. on US 151 just south of Cth DE, in the Township of Calamus.
A preliminary investigation shows Hans Kubik was operating a 2002 GMC pickup truck south on US 151 in the right/outside traffic lane.
John Lighthart, 53, from Rockford, IL was operating a 2017 Peterbilt semi-tractor with box trailer, traveling south on US 151.
The front of the Peterbilt struck the rear of the GMC pickup truck, causing the pickup truck to go off of the roadway into the right/west ditch and overturn.
Kubik was transported to Beaver Dam Hospital and then flown by Flight for Life to UW Hospital in Madison. A passenger in the pickup truck, Mary Larson (68 years old, from Columbus, WI) was transported to Beaver Dam Hospital and then transferred by ambulance to UW Hospital in Madison. Lighthart was uninjured in the crash.
On 05-01-2017, Hans Kubik died at UW Hospital, Madison, WI. The crash remains under investigation by the Dodge County Medical Examiner and the Dodge County Crash Investigation Team
